Polycystic Ovary Syndrome

Menstrual irregularity (such as amenorrhea or oligomenorhea),

body hair growth,

and the presence of small ovarian cysts on ultrasonography-polycystic ovaries (12 or more, cysts of 2-10 mm in size, it is enough to be in one ovary).

Presence of two of the three above-mentioned findings makes the diagnosis of polycystic ovary syndrome. However, for the diagnosis of polycystic ovary syndrome in adolescence, all three of the above-mentioned findings must be present. Patients may have very different phenotypic (external) appearances. These patients are generally overweight, there is a disproportion between height and weight. In the differential diagnosis, hormone analyzes (such as FSH, LH, TSH, PROLAKTIN, FREE TESTORONE, 17-0H PROGESTERONE) and ultrasonography are used. Its incidence in the reproductive population is around 6-10%.

Patients may come with different complaints according to age groups. There is an ovulation problem in polycystic ovary syndrome, which causes infertility, menstrual irregularity, hair growth, acne- increased risk of uterine and breast cancer, increased risk of cardiovascular disease and increased risk of diabetes. Since polycystic ovary syndrome is likely to be associated with diabetes (hyperinsulinism), patients should be checked in this regard as well. Treatment is done according to the patient's complaint, if a young patient only complains of menstrual irregularity, progesterone hormone is given in the treatment, if there is hair growth, birth control pills are given. These treatments are used for at least 6 months and this period can be extended further after the control, it should be considered as a chronic disease.

If a woman wants to become pregnant; First, it is encouraged to be in normal weight, then the ovaries are stimulated with pills containing clomiphene or letrozole and IUI is done. The ovary is stimulated with gonadotropin (FSH) injections and IUI is performed, and if pregnancy cannot be achieved, in vitro fertilization treatment is performed. In polycystic ovary syndrome, before gonadotropin (FSH) needle treatment, ovaries can be drilled with laparoscopy, and after this procedure, it is possible to get pregnant without drug treatment.
