If both tubes are blocked or there is a tube problem after the investigations,

If you have advanced endometriosis disease,

If the woman has low ovarian reserve,

if the woman’s age is high,

If there is a long-term infertility history,

If the spermiogram values ​​are bad (for example, the total number of motile sperm is below 5 million),

If there is unexplained infertility,

If pregnancy has not occurred after IUI treatment, IVF may be beneficial.

 

In vitro fertilization success is 50% on average, and it is a team work. The team of doctors, embryologists, laboratories and nurses are important. The patient's age, ovarian reserve, infertility duration- (it is negatively affected as it gets longer), previous pregnancy status, existing disease (endometriosis or unexplained infertility or polycystic ovarian syndrome), male factor, every stage of the treatment is important for success.

 

After deciding on in vitro fertilization and performing the necessary examinations; patients are included in the program, drug protocols, doses are adjusted according to patient age, weight and ovarian reserve or previous treatment doses. In the treatment, gnrh analog called long protocol or gnrh antagonist protocol called short protocol is applied.

 

In the long protocol, patients start treatment on the 21st day of the previous menstrual period, the patient is called for control on 2 or 3 days of their menstrual period, and if appropriate conditions are met, the ovaries are stimulated with various drugs until the required number and size of eggs are observed (one or two eggs are the aim of IUI while many eggs in in vitro fertilization), an egg maturation injection is made and ovarian stimulation is terminated. In the short protocol, the patient is called for control for ultrasonography and hormone analysis on 2nd or 3rd days of the menstrual period, if the conditions are suitable, the follow-ups continue like the long protocol.

 

34-36 hours after the maturation injection, the patient is called to collect the eggs, the eggs are collected after a short procedure under general anesthesia, the patient is discharged after resting for a while. At this time, the quality of the eggs collected is checked whether they are at the required maturity or not. And in about 2-3 hours, each mature egg is deposited between the sperm (50000-100000) obtained from its partner and processed before egg collection (IVF) or about 12-18  hour is expected for fertilization by placing a sperm inside each egg (ICSI)  .

 

After the egg retrieval day, the patient is started on hormone drugs containing progesterone to until the menstruation or 8-10th week of pregnancy. In the meantime, other drugs can be given according to the patient's condition and the protocol applied. Fertilized eggs are evaluated according to the condition and quality of the embryos and transferred into the uterus on the 2-3rd day or the 5th day. If the woman is younger than 35 years old, one embryo is transfered and two embryos are transferred, if the woman is older than 35 years, the remaining embryos can be frozen according to the patient's request.
